Practice Audit Questionnaires

Patient Satisfaction Questionnaire (PSQ)

The Patient Satisfaction Questionnaire activity provides you with the opportunity to confidentially and accurately assess, reflect and evaluate patient experiences and expectations around practice locality, staff, general organisation and the consultation itself. The results of the questionnaires will be provided to you for evaluation and worksheet completion.

To complete the quality cycle, it is suggested that you re-distribute the questionnaires after 4 months and compare the results to those from before. The PSQ order form and details of CPD Points are outlined in the Questionnaire: Patient Satisfaction Questionnaire (PSQ) (PDF).


Referring Doctors Satisfaction Questionnaire (RDSQ)

This activity provides you with the opportunity to confidentially and accurately assess and reflect on referring doctors' satisfaction with your practice. The results of the questionnaires will be provided to you for evaluation and worksheet completion.

Post Operative Satisfaction Questionnaire (POSQ)

This questionnaire provides you with an opportunity to assess and reflect on the quality of patient experience in regards to the pre- and post-operative care you provide. The results of the questionnaires will be provided to you for evaluation and worksheet completion.

To complete the quality cycle, it is suggested that you re-distribute the questionnaires after 4 months and compare the results to those from before. The POSQ order form and details of CPD Points are outlined in the Questionnaire: Post Operative Satisfaction Questionnaire (POSQ) (PDF)


Self-adapted Patient Satisfaction Questionnaire (SAPSQ)

This sample questionnaire has been developed to assist Fellows who wish to design and administer their own practice-specific patient satisfaction questionnaires.

This activity aims to provide you with a confidential and accurate method of assessing patient expectations and satisfaction with your practice. Following completion of the survey, results should be evaluated and a follow-up plan designed and implemented if required.
